LifeLink DNA Policies and Procedures

1. Client Interaction Policies

  1. Appointments:

 

  • Scheduling: Appointments can be scheduled via phone, email, or through the website. Walk-ins are not accepted due to the confidential nature of the services.
  • Same-day/Last-minute Availability: Clients may schedule same-day or last-minute appointments based on availability.
  • Rescheduling: Clients must notify the office at least 24 hours in advance to reschedule without incurring a fee. Exceptions can be made for emergencies.

 

  1. Confidentiality:
  • Client Privacy: All client information, test results, and interactions are kept strictly confidential. Information is only shared with authorized individuals as necessary.
  • Private Office: Only one client is seen at a time to maintain privacy, confidentiality, and a personalized experience.

 

  1. Reception Experience:

 

  • All clients are greeted by a professional receptionist to ensure a welcoming and comfortable experience.

 2. DNA Testing and Collection Procedures

Collection Method:

 

  • LifeLink DNA exclusively uses mouth swabs for DNA sample collection, whether in-office or through mobile services.

 

  1. Home Test Kits:

 

  • For Informational Use: Clients opting for home test kits will receive detailed instructions on proper sample collection and return.
  • Customer Support: Assistance is available for clients who have questions regarding kit usage.

 

  1. Sample Handling:

 

  • FedEx Shipment: Samples collected in the office or via mobile service will be sent to the lab on the same day if collected within an hour of FedEx’s closing time of 7 pm. Samples collected after 6 pm may possibly be sent the following day.
  • Chain of Custody: Chain of custody procedures are followed for legal cases to ensure the integrity of the sample collection and testing process.

 

  1. Legal Testing Requirements:
  • Clients undergoing legal DNA testing must provide a valid driver’s license or identification at the time of testing and when picking up results. Results must be received in person by the individual who provided the identification. If the results are not picked up within a reasonable timeframe, they will be sent via certified mail to the individual who paid for the test.

 

 3. Mobile Services Procedures

  1.   Mobile Services Procedures :
  • Mobile services are offered throughout Central Florida with flexible hours, including weekends. Clients can choose a convenient location for DNA sample collection.
  1. Equipment and Safety :
  • Mobile collectors will bring all necessary equipment, including sterile mouth swabs and packaging. Proper hygiene and sanitation protocols will be strictly followed for all mobile services.

4. Drug Testing and Fingerprinting Policies

  1. Drug Testing:
  • Clients must present a valid ID before drug testing.
  • LifeLink DNA adheres to all legal guidelines for proper drug testing collection and reporting.
  1. Fingerprinting Services:
  • Secure Recordkeeping: All fingerprint records are securely stored in compliance with relevant state and federal regulations.

5. Payment Policies

  1. Payment Methods:
  • Payment is due at the time of service. Accepted payment methods include credit/debit cards and digital payment systems. Square is the preferred payment processor.
  1. Refunds:
  • Refunds are not provided for completed tests. However, clients who cancel their appointments with 24 hours’ notice may reschedule without additional fees. LifeLink DNA will absolutely honor a refund for any issue that occurs due to our fault and cannot be resolved on our end.
  1. Mileage for Mobile Services:
  • For all mobile appointments that require travel over 30 miles, a standard mileage rate of $0.56 per mile will be charged upfront. This fee is non-refundable if the appointment is not canceled or rescheduled within 24 hours of the scheduled appointment time.
  • An invoice will be sent to clients for the mileage fee prior to the appointment, and this invoice must be paid within 24 hours of the scheduled appointment time.
  1. Payment Plans:
  • LifeLink DNA offers a simple payment plan that allows clients to break their payment into two installments. Clients must make the first payment before the service is provided. Results will not be released until full payment has been received.

6. Referral Program Procedures

  1. Referral Bonuses:
  • LifeLink DNA offers a $25 referral bonus for every completed appointment referred by a client.
  • There is no limit to the number of referrals a client can make, and top referrers may receive additional bonuses or gifts based on the volume of successful referrals.

7. Review and Incentive Policies

  1. Customer Reviews:
  • Clients are encouraged to leave feedback after their appointment. Incentives may be offered for honest reviews of the services.

8. Coaching and Training Procedures

  1. Training Sessions:
  • LifeLink DNA offers coaching for individuals interested in launching their own DNA testing business. Sessions are structured and include detailed training materials, as well as ongoing support post-training.

9. Health and Safety Policies

  1. Sanitation:
  • All equipment used for DNA, drug testing, and fingerprinting is sanitized between clients. Hand sanitizer and masks are available for both in-office and mobile appointments.

10. Compliance with Legal Regulations

  1. Certification and Compliance:
  • All DNA testing, drug testing, and fingerprinting services comply with relevant state and federal regulations.
  1. Documentation:
  • Documentation for all services, particularly for legal or government-affiliated cases, is maintained following strict confidentiality and chain of custody protocols.
